This presentation focuses on two types of interface issues that Process Hazards Analyses often fail to capture: tanker truck loading/unloading of highly hazardous materials into/from a PSM-covered process, and the consequence from one unit that becomes a cause in a receiving/ downstream unit. Examples of both will be presented, accompanied by explanations for the failures to properly assess the issues and means to address them.
