Over the last few decades, great strides have been made to identify, address, and eliminate the causes of industrial and process safety incidents. Despite these improvements, many of our friends and co-workers are still getting injured and process safety events are still too commonplace. As we target (and approach) a zero incident culture, the truly hard work has begun. With this in mind, "What's in the Gap?" is intended to clarify what lies between "here and zero" and to strengthen our resolve to press on toward a day in which everyone goes home safely everyday and process safety events are a thing of the past.
