What do we really know about SIFs? What we are doing isn't working; new technology is not helping; training and skill development hasn't touched the problem The point is, we have to do something different! Gary will introduces some newly discovered factors and possible solutions to assist us in the battle against SIFs. There is no 'magic pill' to make this issue go away but there are some activities we can do that if combined with current efforts, they may make risk more visible and allow for more effective solutions.
